Tonya Flaming RN
Tonya Flaming is a registered nurse with several years’ clinical experience working with children as a discharge planner in the areas of trauma, neurosurgery, neurology, intensive care, and burns and plastics.
Prior to joining Rehabilitation Planning in 2012, Tonya worked at Toronto’s Sick Kids Hospital for 14 years, with the last six spent as a Transitional Care Coordinator/Discharge Planner.
Tonya is actively involved in the brain injury field, sitting on both the Brain Injury Society of Toronto's Awareness Committee and the ABI Network Conference planning committee.
Tonya was the recipient of the 2017 Award of Excellence
in Brain Injury Rehabilitation as Case Manager of the Year.
Leigh Craney Bsc.
Leigh Craney holds a Bachelor of Science degree from Brock University and has been working as a community case manager for the past 14 years. While working it the field, Leigh has successfully completed extensive brain injury and spinal cord injury educational training through the Ontario Brain Injury Association (OBIA) & Brock University, McMaster University, and the University of Calgary. Leigh works in a collaborative fashion to coordinate comprehensive community rehabilitation programs for both catastrophic and non-catastrophic clients with acquired brain injury, spinal cord injury, amputation, chronic pain, addiction and complex mental health diagnoses.
Leigh is an accredited Registered Rehabilitation Professional (R.R.P.) through the Vocational Rehabilitation Association of Canada and a VRA member in good standing
Leigh joined the Rehabilitation Planning team in 2003.
Judith Suraski RN
Judith Suraski is a registered nurse with more than 40 years of clinical experience in intensive care, coronary and post-coronary care, ambulatory urology services, and maternity care. Judith has extensive experience working both with pediatric and adult patients and has worked in case management since 1988. Prior to joining Rehabilitation Planning, she worked as a case manager at a Community Care Access Centre (CCAC) for 15 years.
Judith joined the Rehabilitation Planning team in 2004.
Laurian Nowitz PT
Life Care Planner
Laurian Nowitz is a Registered Physiotherapist and Certified Life Care Planner through the Commission on Healthcare Certification (CHCC). She performed Functional Ability Evaluations for Medical and Rehabilitation, Post-104 Disability and CAT DAC Assessments. She has 26 years of experience in assessing and treating children and adult clients with birth injuries (including cerebral palsy and brachial plexus injuries), head injuries, spinal cord injuries, and orthopedic injuries, multiple trauma, amputations and chronic pain. Her clinical experience includes treatment of children with cerebral palsy using a variety of techniques including neuro-developmental treatment approaches, MEDEK, Bobath, Constraint Therapy.
Colleen Lewis is a Registered Nurse with over 25 years extensive background in nursing both in Canada and the US.
Her professional experience includes providing comprehensive case management services to clients with catastrophic impairments such as: traumatic brain injuries, spinal cord injuries, chronic pain, and multiple impairments.
Colleen also serves as a Legal Nurse Consultant, providing assistance to lawyers and legal professionals, with expert legal nurse consultation and opinions on medical malpractice cases.
